We are excited that you have chosen Catawba Pediatric Associates for your child's pediatric medical care.
New patients will need to have medical records transferred to us from their current provider. Once these records are received by our office, we will call you to set up wellness visits. In addition to the medical records we will need a immunization record and insurance cards. Once an appointment is scheduled you will receive emails regarding paperwork that needs to be completed. The paperwork and virtual check in will be done through and online feature called Phreesia. If you have questions or need help doing this please call our office. Once established, you may also request an appointment or register for a future appointment via our patient portal. If you're preparing for your child's first visit, you can do a few things to help expedite the appointment. Please provide the following information when you arrive for your visit:
- Insurance cards
- Complete immunization records
- Medical records from your previous pediatrician
New patients will need to fill out a form to have records transferred to us from your current provider. You can complete the form through us by coming by our office, printing the form and giving it to your current provider to send records. Please be sure to bring your insurance card and photo ID to every visit. We look forward to meeting you!
